Enrollment Terms & Conditions
These terms apply to all enrollments at Ormond Ballet. By completing the enrollment form, you agree to each of the following policies.
1. Tuition Invoicing
After completing the enrollment form, a recurring invoice for tuition will be created for the account. Total tuition owed for the current semester will be calculated and a payment plan will be created on a monthly basis unless otherwise agreed upon.
- Billed Weeks: Whole weeks tallied from your start date through the last day of the semester. Any designated studio closure weeks are excluded and are not billable. Billed weeks will not be subdivided.
- Semester Tuition: "Weekly Tuition" multiplied by "Billed Weeks."
- Number of Invoices: You will pay forward for the classes enrolled. For example, if the semester runs 5 months and you start in the first month, you will have 4 invoices covering the classes across all 5 months. If you start in the second month, you will have 3 invoices, and so on.
- Amount per Invoice: "Billed Weeks" divided by "Number of Invoices."
2. Payment Methods
Ormond Ballet practices a PAY FIRST policy. Invoices will be structured so that the weeks charged are always in the future. When completing your first payment online, you will have the option to set up automatic payments. We highly recommend this.
REQUIRED: SAVE YOUR CARD ON FILE. When making your first payment you will have a checkbox that gives us permission to save your card on file. This box must be checked. We use Square Inc. as our payment processing system, which encrypts your card information. No employee of Ormond Ballet has access to your card information.
Ormond Ballet employees can and will charge the card on file on all invoices that are 7 days past due.
You must communicate with us if there is a financial issue, as we need to receive payment in order to run our business and serve your students. If you cannot save a credit card on file, a written agreement will be required and payment must be made by cash or check on a date agreed upon each month.
3. Class Withdrawal & Modification
Class Withdrawal: Withdrawing from all classes for a semester results in the student(s) no longer being enrolled at Ormond Ballet.
14-Day Notice Policy: A minimum of 14 days written notice is required before the desired withdrawal date. The account holder remains financially responsible for all tuition during the notice period regardless of attendance. Withdrawal requests submitted with less than 14 days notice will not be accepted.
The final tuition will be determined by the total number of weeks from the starting week through the withdrawal date week. The account holder is financially responsible for all weeks from start date to withdrawal date. Final payments must be made on the written notification date or a minimum of 3 weeks prior to the withdrawal date. If the account holder has paid for more than the total number of weeks calculated, they are entitled to a prorated refund. Note: Weekly tuition will not be subdivided, regardless of attendance.
Class Add/Subtract: Adding or removing classes adjusts the student's weekly tuition accordingly.
4. Attendance
Attendance will be taken by the teacher of each class.
- Missed classes: It is the student's/parent's/guardian's responsibility to make up any missed classes and to inform Ormond Ballet. No refunds or account credits will be issued unless approved by the Director.
- Late for class: The student/parent/guardian is responsible for informing Ormond Ballet of any tardiness.
5. Release of Liability
Ormond Ballet will not be held liable for any injuries or property damage.
The enrolled participant(s) and/or parent(s)/guardian(s) recognize and understand that dance, fitness training, physical theater, and technical production are potentially hazardous activities and that there are risks inherent in these activities, including but not limited to serious physical injury.
I/we hereby agree to indemnify and hold harmless Ormond Ballet, its instructors, officers, directors, agents, and employees against any liability resulting from any injury that may occur to the participant while participating in activities of Ormond Ballet.
6. Medical Emergency
The participant(s) and/or parent(s)/guardian(s) agree to indemnify Ormond Ballet for any damages incurred arising from any claims, demands, actions, or courses of action by the participant.
I/we authorize any representative of Ormond Ballet to seek medical treatment for the participant if needed during his/her participation in activities of Ormond Ballet.
I/we agree to pay all costs associated with medical care and transportation for the participant.
Any special medical or health concerns — including allergies, physical/mental/emotional restrictions, injury history, learning differences/special needs, or medical emergency information (such as EpiPen or inhalers) — that staff should be aware of must be shared via a written statement prior to participation.
7. Disclaimer & Consent (Hands-On Teaching)
All dance instructors at Ormond Ballet use a hands-on teaching method. In order to facilitate the student's learning, the teacher may occasionally physically guide a movement or correct body placement by touch. This will always be done in a professional manner and the intention of the contact will be made clear.
The participant(s) and/or parent(s)/guardian(s) understand this approach is a helpful teaching and learning tool and consent to its use by instructors during the participant's lessons and rehearsals.
8. Illness & Injury
Students who are ill (infectious) should not come to class and will be sent home if they attend. Do not send your student to the studio if there is any reason to believe they are not feeling well. Students can make up missed classes due to illness if Ormond Ballet is properly notified ahead of time.
Injured dancers are encouraged to observe class in order to remain familiar with the curriculum. Students will be asked to write their observations and give them to the instructor after class. If an injury prohibits a student from attending, they may make up missed classes if properly notified ahead of time.
There will be no refund issued for missed classes.
9. Equal Opportunity Policy

10. Media Release (Optional)
The enrolled participant(s) and/or parent(s)/guardian(s) may authorize Ormond Ballet and/or its representatives, agents, or employees to photograph and/or video record and use any photograph or likeness of the participant for publicity, choreographic archives, and promotional materials, both online and printed. This section is optional and consent is given separately during the enrollment process.
